The Heart Foundation will soon be launching a Heart Health Check Toolkit for GPs, practice nurses and practice managers.

Don’t miss out on access to the Heart Health Check Toolkit

The toolkit is a one-stop shop for information, tools and resources to help you implement Heart Health Checks at your practice in the simplest and most impactful way possible.

You will have free access to templates, factsheets, posters, quality improvement activities and much more. 




The National Heart Foundation of Australia acknowledges the support of the Victorian Government in the development of the Heart Health Check Toolkit.
