As a general practitioner, we would like to invite you to participate in a survey that we are conducting with general practitioners across Australia.
What is the survey about?
This survey has been designed to collect feedback from general practitioners about their awareness and use of Heart Foundation materials and key messages, as well as gaining a better understanding of their interaction with clients and the practices they employ.Collecting this information will assist us in better understanding general practice, and help us improve our communications to assist health professionals in diagnosing and supporting patients with heart disease.
How to complete the survey
